PurposeMetabolic / Weight loss
MechanismGLP-1 receptor agonist: increases satiety, slows gastric emptying, improves insulin secretion
EffectsLess hunger, slower digestion, better blood sugar; often improves BP and overall CV risk
DosingInjection: 0.25 mg weekly, stepped up every 4 weeks through 0.5, 1 and 1.7 mg to a maintenance of 1.7 or 2.4 mg weekly; maximum 7.2 mg weekly for adults who have tolerated 2.4 mg for at least 4 weeks (Wegovy, label 06/2026). Wegovy tablets: 1.5 mg, then 4 mg, then 9 mg, then 25 mg once daily, in 30-day steps.

Cautions & interactions

GI side effects; risk of pancreatitis; gallbladder disease; contraindicated in MTC/MEN2 history

Caution with insulin/sulfonylureas (hypoglycemia risk); delays gastric emptying may affect absorption of oral meds; monitor for dehydration if on diuretics.

CV 5 of 5 — Strong human outcomes data via diabetes/obesity trials; improves multiple CV risk factors (weight, glucose, BP).
