PurposeMetabolic / Weight loss
MechanismTriple monoamine reuptake inhibitor, raising dopamine, serotonin and noradrenaline. Not a peptide and not an incretin — a centrally acting appetite suppressant.
EffectsPhase 2 mean weight loss of 4.5% at 0.25 mg/day and 9.2% at 0.5 mg/day.
DosingNot FDA-approved. Approved in Mexico since 2023 as Nupenta (monotherapy) and Tesomet (with metoprolol). Phase 3 planning used 0.25 and 0.5 mg/day orally.
Bottom lineReal weight-loss data and a real cardiovascular liability, which is why it is sold with a beta blocker where it is approved at all.

Cautions & interactions

Monoamine reuptake inhibition raises heart rate and blood pressure, which is why the Mexican combination product pairs it with a beta blocker. Not approved in the US.

Serious interaction risk with MAO inhibitors, SSRIs, SNRIs and other serotonergic or sympathomimetic agents. Coadministration with monoamine-active drugs needs specialist oversight.

CV 1 of 5 — Raises heart rate and blood pressure through monoamine reuptake inhibition. The approved Mexican combination adds metoprolol specifically to offset this. Treat as a cardiovascular liability, not a neutral agent.
