PurposeMetabolic / Weight loss
MechanismNon-peptide small-molecule GLP-1 receptor agonist taken orally, with no food, water or timing restriction — the property that separates it from oral semaglutide.
EffectsAppetite reduction and weight loss from a daily tablet. In ATTAIN-1, participants on the highest dose who stayed on treatment lost about 12.4% of body weight against 0.9% on placebo.
DosingOral tablet once daily: 0.8 mg for at least 30 days, then 2.5 mg, then 5.5 mg; optionally 9 mg, 14.5 mg and 17.2 mg, each step after at least 30 days, by response and tolerability. Maximum 17.2 mg daily, or 9 mg daily with a strong CYP3A4 inhibitor. Approved by the FDA in April 2026 for chronic weight management in adults with obesity, or overweight with a weight-related condition.
Bottom lineThe second oral GLP-1 to reach market and the first with no food or water restriction. Approved, with real trial data behind it.

Cautions & interactions

Boxed warning for thyroid C-cell tumours. Nausea, diarrhoea, constipation and indigestion are common, worst on starting or escalating, and settle over time. Pancreatitis and hypoglycaemia are less common but need prompt attention.

As a GLP-1 agonist, caution with insulin and sulfonylureas for hypoglycaemia risk. Delayed gastric emptying can affect absorption of oral medications.

CV 3 of 5 — Shares the class metabolic profile — weight, glucose and blood pressure improvement. No dedicated cardiovascular outcomes trial has read out, so the outcome data behind semaglutide does not transfer to it yet.
