PurposeMuscle / Lean-mass preservation
MechanismMonoclonal antibody against type II activin receptors, reducing fat mass while promoting muscle growth. Same receptor family as ACE-031, but an antibody rather than a decoy receptor.
EffectsIn the BELIEVE Phase 2b trial, bimagrumab with semaglutide reached up to 17.8 kg weight reduction against 14.2 kg for semaglutide alone, and largely preserved lean mass where semaglutide alone reduced it. At 30 mg/kg alone, lean mass rose slightly above baseline.
DosingNo human dose established.
Bottom lineThe most credible answer so far to the muscle-loss problem in GLP-1 weight loss, and still Phase 2b.

BELIEVE used 10 or 30 mg/kg at weeks 4, 16, 28 and 40 over 48 weeks, alone or with semaglutide at 1.0 or 2.4 mg weekly.

Cautions & interactions

Investigational; Phase 2b only. The activin receptor pathway is the same one whose blockade ended the ACE-031 programme for bleeding effects, though bimagrumab has not shown that signal.

Studied in combination with semaglutide. No formal interaction studies otherwise.

CV 2 of 5 — Lean-mass preservation during weight loss is favourable in principle. No cardiovascular outcomes data, and the related ACE-031 programme ended on vascular effects.
